REGISTRATION
To register for the Diabetes Self-Management Program, contact Community General Hospital’s Health Education Department at 468-1997. A physician referral is needed to register, and a referral form is included in this brochure for your convenience.

FEES
Participants are encouraged to check their individual insurance plans regarding coverage of the program. Third party coverage is often available, and Community General Hospital’s program is certified by Medicare for reimbursement.
